This is the Kindness For K-9’s Rescue adoption agreement

1. Before completing this application please read it carefully know what you are agreeing to.

2. You will need to meet the transporter if your animal is still in Kentucky. The transporter has stops Hagerstown,MD; Allentown,PA; Harrisburg,PA; Gouldsboro,PA; Parsippany,NJ; Newburgh,NY; Danbury,CT; West Hartford,CT; Union,CT

3. Please have all family members in agreement to adopt.

4. Please hold off on applying if you will be having delay to pick out your animal as dogs change quickly and it’s unfair for the animal to miss another good home.

5. All animals going to their forever homes will come micro-chipped, spayed/neutered and medical records that show the vaccinations given.

All animals are spayed or neutered before being adopted. They have appropriate shots and worming for their age and have been wormed at least two times. Folder with Medical records comes with each animal. I agree that all statements I have made on this form are true.
